Living with Epilepsy Survey

If you have epilepsy or a seizure disorder, the Department of Health wants to know. Visit their website at www.doh.state.fl.us and complete a statewide needs assessment for people living with epilepsy. The survey is being conducted in an effort to ensure that the Epilepsy Program of the Florida Department of Health and the local epilepsy foundations/associations better serve people with epilepsy or a seizure disorder.

The survey instrument is located at www.doh.state.fl.us. From the website, select "Epilepsy" from the drop-down box, then click on "Living with Epilepsy." A summary of survey results will be compiled and posted on the website at a later date. For more information, call (850) 245-4330.
